RESPONSIBILITIES:
• Comprehensive Program Development: Ensure standardized program development leveraging best practice and measurement-based care. Create program operations manual to establish baseline expectations for program fidelity.
• Support Action Teams with Data Expertise: Align program design with key performance indicator (KPI) development and measurement based-care outcome targets in partnership with Oxford OHT.
• Collaborate with Partner Organizations: Coordinate with existing mental health and addictions service partners to ensure cohesive pathway development and alignment of processes across partners. Guide and lead the co-design process and activities to lead the development of the new program, informed by both those with lived experience as well as providers.
• Create Data Visualizations for Program Development: Utilize data visualization techniques to present findings clearly, supporting informed planning and decision-making among Oxford OHT stakeholders.
• Stakeholder Engagement: Conduct regular partner focus groups to identify key areas for standardized assessment, intake, treatment, and discharge processes, including acuity measurement for to address client specific level of service needs.
• Engage with Regional Mental Health and Addictions Providers: Represent Oxford OHT in external working groups, webinars, and sessions; collaborate with neighboring OHTs to foster knowledge-sharing and report key insights. Ensure development of pathways for equity deserving groups through consultation and engagement of local experts.
KEY COMPETENCIES:
• Mental Health and Addictions Treatment Expertise: Skilled clinician with experience in both mental health and addictions treatment pathways from intake to discharge.
• Program Development and Organizational Skills: Skilled in developing program operation manuals which are inclusive of multiple stakeholders / partner needs for service integration.
• Exceptional Communication: Strong written, verbal, and interpersonal skills, with demonstrated ability to engage a diverse group of stakeholders, including patients, providers, and community partners.
• Presentation and Engagement: Skilled in presenting complex information in a clear, easy to understand and engaging manner and, acting as a resource for others.
• Relationship Building and Influence: Proven ability to foster partnerships, influence others, and effectively share ideas and perspectives.
• Technical Proficiency: Proficient in the use Microsoft Office 365 (Power BI, PowerPoint & Microsoft Word).
• Data Analysis: Demonstrated ability to collect, analyze, and apply quantitative and qualitative data insights for decision-making.
• Healthcare Knowledge: Strong understanding of Ontario Health Teams, and healthcare and social service systems in Oxford County.
• Administrative: Collect, record and update client/program statistics. Maintain records according to organization standards. Maintain positive interpersonal working relationship and adheres to corporate core behavior. Actively participate in supervision, professional development and self-assessment aimed at developing/enhancing own professional skills
QUALIFICATIONS:
EDUCATION
• Master's degree in Social Work from an accredited University
• Project Management and/or change management experience is considered an asset.
• Previous experience working in a publicly funded health care organization (i.e., acute care, primary care, mental health, and addictions, etc.)
• Knowledge of data collection requirements and methods (e.g., InfoMed, Greenspace, DAD, NACRS, NRS, OHMRS, IDS, etc.) considered an asset.
• Experience working with or in interdisciplinary teams.
REGISTRATION
• Membership in the Ontario College of Social Workers and Social Service Workers or the College of Psychotherapists of Ontario
